Thank you for choosing our office for your urology specialist. This is a full disclosure of our financial policy. We hope this will answer any questions you might have. If not, do not hesitate to ask the receptionist.

You must present your current insurance card(s) to the receptionist at each visit. If you have more than one insurance policy, please be sure to tell the receptionist which is primary and which is secondary. If we have a problem with the insurance paying your claim we may ask for your assistance.

Co-Payments, deductibles and Co-Insurance Amounts are due at the time of service. This is required by the insurance carrier under our contract. We accept cash, check, debit, American Express, Visa, Discover and Mastercard.

Our providers participate with most insurance plans. Please call your insurance carrier and make sure we are participating with your plan. If you need a referral, it is your responsibility to obtain it from your primary care doctor. You must have the referral at the time of your appointment or have it faxed directly to our office. If you do not have a referral your appointment will be re-scheduled.
